Technically, A "vanilla" server is a completely unmodified server. However, since the release of Bukkit, nobody (in my knowledge) still uses Notch's code for running a server.
Now, a "vanilla" server has come to mean any server that has no mods that effect gameplay. This means that if a server has a griefer-rollback plugin, such as Big Brother, this server can still be considered a vanilla server, because Big Brother doesn't alter how one could play on that particular server.
Servers can be considered "vanilla" if they play like a true "vanilla" server, even if there are some modifications to how the server is run.
