Just wanted to update:
I'm not having this issue anymore. It looks like the server was just overloaded and by removing a couple mods (enderIO and endercore) the server runs smoothly and without problems.

I'm using forge 1.12.2 and my server has been stopping after 10-15 minutes of game. It's been happening for 2 days and I have no idea why.
