Check if you have the same problem with a new world.

Remove the addons one by one from your server.
Every time you remove a mod/plugin from your server, you need to restart it, then check if the problem still occurs.
If the problem doesn't occur anymore, know that it was probably due to the mod/plugin you last removed.

Either it's a bug in Dimensional Pockets II or it's missing an dependency / update of them

It looks like your server is running out of RAM.
Try to remove plugins/worlds. It could also be caused by a single plugin which is buggy
