Make sure your graphics drivers are up-to-date. Did you try reinstalling the mod yet?

Can you post your crash log?

Do you use a custom world on your server? Are you able to open a single player world?

If even single player mode doesn't work either the mod itself doesn't work or you installed it falsely.
