    Hi, I would help but I actually need more info on how this happens, does this happen on a map, or is it just vanilla minecraft? (Command blocks might be the problem)

    Do you have any plugins, if so what version is your server, and what is the plugins version, if the plugin is outdated, it might not work well with the server.

    Check the logs, too, it'll show the errors, if there are any.
