Ranks & Op settings reseting

HI! So first of all the thing is every time I log in to my server the OP setting always resets. Is that how it works? or there's a problem?

The second is, I am using the PowerRank plugin and the same with the first problem every time me and my friend re-login to my server it always resets to default/ which is Member rank.
