Just go to 1.16, it's a non-breaking update, meaning nothing happens to your world (tho you might wanna reset your nether)

Do they show in singleplayer?
Is the server bedrock or pocketmine? -
Did you try /difficulty normal in the nether, then restarting?

It does say ingame there's a new version afaik, but there isn't actually. 4.5.1 is newest

Sorry but what? The server log helps to find issues with the server, and usually the IP is not showed.
He posted his log directly from /log, so his IP was visible. I edited it
