Make sure your permissions.yml doesn't contain any syntax errors. YAML's syntax is pretty strict, check your configuration file using http://yaml-nitrado.appspot.com for example to find the mistakes quickly.

Make sure you installed everything correctly and you join your server using the correct Minecraft and mod version.

Generate a Biomes O' Plenty world in single player mode and then upload it to your server.

Most permission managing plugins including Group Manager and PermissionsEx allow you to manage permissions per-world. Just google how to do it.

I just started and joined your server, it works fine for me.
Maybe there is a problem with your player file though. Download your world, remove the players directory out of it and then upload your world again. Everyone's inventory will be deleted then, but you should be able to join your server again.

The original Essentials Group Manager isn't available neither at DevBukkit nor at SpigotMC, so we can't add it. We have a fork that's called Bssentials Group Manager though, it should work the same way.

It seems like your server takes too long to start. Download and save your world to your computer and reinstall your server to make it work again.

Make sure to configure the plugin correctly. Check the server console log for additional error information.
