Your server is running out of memory so try removing some mods and install ferritecore mod that will help reduce memory usage

Which modpack are you trying to use?

I tried installing the mods you have with forge https://aternos.org/software/v/forge/1.12.2-recommended and mixinbootstrap mod and it didn't crash

Can you share a screenshot of the mods you use and have you tried installing a different version of forge?

I can't see any connection attempt to the server so share your client log once you try joining

Your trying to run forge mods(Pehkui, Extra Origins) on a fabric server so either install the fabric versions of the mods or remove it

Share your client log:
1. open your minecraft installation path (enter %appdata%/.minecraft in the file explorer )
2. open the logs folder
3. open the "latest" or "latest.log" file with a text editor
4. copy and paste the content of the file to https://mclo.gs/
5. send the link here
