So i have been trying to download my world and i keep running into a problem where when i go to download the world its not up to date and its an old save does anyone know how to fix this, i tried /save-all but that didnt do anything

Why do you think it's not?

A fresh download can't contain anything else than the updated world... are you sure you're looking at the correct file. If you downloaded your world more than once and didn't overwrite it, it has a number behind it like this:
CodeTomtaru-world.zip Tomtaru-world(1).zip Tomtaru-world(2).zipLook at the date of your .zip file and make sure it is the most recent. There is no other way I can think of that your downloaded world could be outdated...

What exactly is missing?
Only player related data (inventories, achievements etc.) or also buildings?

This is because your client is using different uuids to identify players than the server.
This can have multiple reasons, either your client is not working correctly or you played too much with the cracked setting on your server.
You would need to manually fix the playerdata of the downloaded world which is very complicated. I would recommend you to just cheat the items on singleplayer.
