Server Crashes when Anyone Dies

I have been having this problem where the server crashes when someone dies. It say's server closing when someone died, and it's real annoying. We have to wait for it to turn off, and then wait for it to turn back on. Our server is modded, but I have no clue how to solve it or what is causing it. If anyone can explain, that'll be great.

These are the logs, but I cannot find when I died. It also said server closed after I did die.
