A plugin doesn't have the proper chunk height-whatever thingy set to 256, it is currently set to 0. By changing it in the files and config, you should be able to fix it. The plugin apparently is "Chunk IO" or something.

You probably started your server in 1.13 and switched back to 1.12.
This is not possible, you can not switch back to 1.12. after starting your server once with 1.13
